Pros: The audio aspect forces you to devote full attention to the information and not be distracted by a visual presentation. Great format, specific articles can then be requested or copied from local sources.
Cons: Graphs would be useful

Cons:
Other thoughts: I still prefer to see a pdf of papers. Rather than expend the effort to provide an audio version, I'd prefer to have the full article made available at no, or at least greatly reduced, costs. I'm a patient, not a healthcare professional. I cannot affort full subscriptions to the many journals covering my disease, nor to pay $25 or more for each paper.

Pros: It is a fantastic tool learning tool. As the Risk Manager of our University Hospital, having a particular interest in all features of NEJM related to treatment and management of disease, beside public health issues, I always encourage hospital staff and students to access the NEJM site, widely publicizing the "Free full text" items available. This is an extraordinary way of not only keeping up to date with reliable information on relevant medical issues, but practicing listening and learning spoken English. Thanks for another great tool.

Reviewer: D. Koper Professional Specialty: Hematology/Oncology Place of Work or Study: Pharmaceutical Company
10/18/2007
Pros: A useful review but only to a limited extent as important information about the patient's history is missing..
Cons: There is no mention that zinc deficiency, toxic metal accumualtion and use of progesterones are common causes of hair loss. Iron therapy without zinc supplementation can cause further zinc deficiency and therefore poor hair growth.
Other thoughts: It is odd to mention use of oral contraceptives as treatment because about 90% of 40 year-old women have a history of use of progestins. Also progesterone cream is widely promoted on the internet. Progesterone has androgenic metabolites and some progestins are strongly androgenic. It is important to test levels of essential and toxic minerals and PUFAs. It is my experience that stopping exogenous progesterones and oestrogens and repleting nutritional deficiencies are basic in the management of hair loss and the prevention of auto-immune diseases..

Pros: Information regarding epidemiology, clinical features and, to some extent, diagnosis are accurate and useful.
Cons: Lack of mention of evidence-based non-pharmocologic treatments for hip OA. Lack of mention of the presence of a "capsular pattern" to help diagnose hip OA.
Other thoughts: A recent (2004) RCT by Hoeskma, et al demonstrated the benefit of manual therapy for the treatment of hip OA. The benefits were maintained at a 29-week follow-up. Several studies have shown the benefits of supervised exercise. I dont' understand why these physical therapy interventions were not mentioned in the article.

Pros: Keeps my attention from wandering, finished the article in good time
Cons: Can be monotonous and boring as there's no visual stimulation
Other thoughts: The video with the Natalizumab article (N Engl J Med 2007;356:2622-9) was brilliant. Would be great if full text reading like this one could be combined with something like that

Pros: It's a great way to go through the NEJM articles on the fly. However all articles , not only the review articles should be abstracted and converted into audio format. Older review articles also can be converted into this format.

Pros:
Cons: The gluten-induced autoimmune diseases were not even mentioned in this article. anti-gliadin antibody is present in at least 1 in 67 women. Gliadin shares common epitope with calreticulin, the major calcium-binding protein. Minimum standard of care in osteopenia, especially with low BMI, must include screening for celiac disease
Other thoughts: Celiac disease is present in 1 in 105 pesons in the US. Osteopenia is present in the majority of those, even children and men. Bone resorptive inhibitors are not indicated in these persons.

Pros: Convenient for high speed access (much less so for those of us who work in rural areas without broadband).
Cons: Can be read in a fraction of the time. Inefficient. Subject to more digital errors.
Other thoughts: While this seems to be an advance, I would find a textual version of the article(s) much more efficient. A useful feature would be to combine audio with PDF.

Pros: Dear Sir / Madam, I regard audio files from The NEJM as one of the most important innovations of your online service. As I compare it with different journals and UpToDate, it helps you stay ahead of tough competition, although many sites have improved their services significantly. I find full audio articles as the most useful audio feature. Summary is also very helpful, for a general review, but full versions should also be available for listeners.
Cons: No full audio versions for all current articles (yet)
Other thoughts: Please continue audio service as it is extremely helpful for keeping in touch with the best what The NEJM can offer. Yours sincerely Cezary Szmigielski, MD, PhD

Pros: An idea whose time has come.
Cons: This is clearly a matter of opinion, but I would prefer the reader take a more dynamic speaking style to act as a kind of audio highlighter.

Pros: This was an excellent tool that allows one to listen to an article while trying to keep pace with a busy schedule. It was well written and chalked full of information.
Cons: None

Pros: The review is excellent, citing both the New Orleans and Canadian Guidelines. What we guess at but don't know about this injury is legion.
Cons: No pediatric section which is the real problem; all children go everywhere, head first. Recent CME indicates that epidemiologic studies 20 years from now will cite us for over-ordering CT scans of every type.

Pros: Easy way to enjoy the articles. Professional voice maintains high quality of presentation.
Cons: Experience would be enhanced if graphs/charts/tables would "pop-up" as they are referenced by the reader.
Other thoughts: Link listening to these presentations with CMEs. Initiate an on-line CME "bank" that would record time logged on and what article was reviewed. I am certain many readers would find this very beneficial for ongoing licensing requirements.

Pros: A novel way of keeping physicians updated with review articles.
Cons: Long duration. Lack of summary points at the end of each section.
